Include with Your Referral:
- Reason for referral
- Assessment and discharge summaries from previous or current providers
- Demographic information
Important: Darby Creek Counseling does not initiate contact with referred clients unless we are provided with a Release of Information. Professionals referring clients are responsible for ensuring their clients are aware they must reach out to us to begin the intake process.
Submitting a referral does not substitute the intake process. Referred clients are required to complete the necessary intake paperwork, available on our website, in order to become clients.
